Summary

This study reports the first case of cutaneous epithelioid angiomatous nodule (CEAN) developing within a pre-existing capillary malformation (CM). The findings suggest CEAN may be a variant of pyogenic granuloma (PyG) with epithelioid features.

Background:

Cutaneous epithelioid angiomatous nodule (CEAN) is a rare benign vascular tumor.
Development of CEAN within a pre-existing vascular malformation is previously unreported.

Observation:

A 52-year-old woman presented with multiple papules on a childhood-diagnosed capillary malformation (CM).
Histopathology revealed CEAN with epithelioid cells (CD31+, CD34+) and associated CM.
Other lesions showed pyogenic granuloma (PyG) with epithelioid features and CM.

Findings:

This case represents the first documented instance of CEAN arising in a CM.
Immunohistochemistry confirmed epithelioid cell morphology and pericyte involvement.
Co-occurrence of CEAN, PyG with epithelioid features, and CM was observed.

Implications:

Suggests CEAN may represent a variant of pyogenic granuloma (PyG) with predominantly epithelioid morphology.
Highlights the potential for CEAN development in pre-existing capillary malformations.
Expands understanding of the histopathological spectrum and potential origins of CEAN.
