Summary

Syringe exchange programs (SEPs) in the US show effectiveness comparable to international programs. Participants reported reduced unsafe injections without increased drug use, though higher initial risk factors were noted.

Background:

Uncertainty exists regarding the generalizability of European and Australian syringe exchange program (SEP) findings to the US context.
Existing studies indicate SEPs promote safer injection practices and do not increase drug use.

Purpose of the Study:

To evaluate the operations and effectiveness of the longest-running SEP in the US.
To compare the outcomes of a US-based SEP with international studies.

Main Methods:

Analysis of data from 204 participants in the Tacoma, US-based SEP.
Comparison of participant injection frequency and safety before and during program participation.
Cross-study comparison with European and Australian SEP research.

Main Results:

Participants reported no significant change in injection frequency (155 to 152/month).
A significant decline in unsafe injection frequency was observed (56 to 30/month).
Higher initial rates of drug injection, unsafe injection, and homelessness were associated with continued unsafe injection, indicating a need for additional services.

SEPs can reduce unsafe injection practices without increasing illicit drug use.
